Although the current healthcare system faces monumental challenges, and nothing can replace human touch in the home, we have great news for Columbia County Residents. There is a solution offered by EverHome Columbia, Inc. that incorporates revolutionary new technology, powered by VivaLynx, with individualized care coordination to provide a lifeline for older adults and persons with disabilities who want to continue to live safely and independently in their own home.
Through a generous subsidy provided by the Home for the Aged, a Columbia County foundation focused on helping local seniors, this Program is being offered for FREE to the first 50 Columbia County residents who apply. The Program will then be available with a sliding scale to subsidize costs for financially eligible applicants.
The Columbia County Office for Aging, through funding from New York State, is providing additional services to assist Columbia County residents in enrolling in the Program and coordinating services. Other community partners include Columbia Memorial Hospital, Columbia County EMS, the Health Care Consortium, Columbia County Department of Social Services and Columbia County Chamber of Commerce.
Participants in the Program will receive:
- Free in-home assessment
- A personalized Care Plan
- Virtual caregiving tools provided by VivaLynx that help identify potential problems before they become a crisis
- Connection 24×7 to family caregivers, care coordinators, and emergency Telehealth physicians
